Emergency Medical Form---Please submit!
For the safety of all children enrolled in the Darien Summer School, all hand-delivered, mailed, or faxed registrations must be accompanied by the emergency medical form, that appears on page 18 of the printed catalog.

If you do not submit the medical form, we cannot complete your registration. You will be notified by email or telephone as quickly as possible about the incomplete or missing form. Your registration will then be set aside until such time as the completed medical form is received. At that time your registration will be complete and processed.

Completed registrations will be given first consideration and processed as quickly as possible.
